Outpatient Check-in Checklist

YOUR ROLE:

Your clinic or health organization may give you the option to check-in online before your appointment which may significantly lessen your wait time or even completely eliminate the need to stand in line for the usual check-in.

 

WHAT TO EXPECT:

Upon arrival, you will spend just a couple minutes being interviewed by non-medical staff which involves verifying your identity, so it can be linked to the proper medical record. This is an important step in your participation to ensure that you have a safe care experience. If you have never been there before, a blank medical record will be started, but if you are a returning patient, you will be linked to your historic record. You can expect to answer questions typical to registration processes such as name, date of birth as well address verification and request for government issued ID cards.

 

KEY TO QUALITY (KTQ):

This is a key safety step because your historic records can provide them with information that will help them double check your historic allergies with any you report.

YOUR ROLE:

Ensure they have identified the correct “John Smith”.  At this point you may also ask if the physician is on time or running late with his/her appointments, so you can plan for a short or longer wait time.
